Background: Erythroid maturation agents offer transfusion independence for MDS-RS patients. Methods: Long-term analysis of 140 lower-risk MDS-RS patients on luspatercept 1.0-1.75mg/kg every 3 weeks for up to 48 weeks. Results: Erythroid response achieved in 63.5% of evaluable patients. Transfusion independence (≥8 weeks): 38.2%. Median duration of erythroid response: 30.6 weeks. SF3B1 mutation associated with superior response (OR 2.8, p=0.003). Conclusion: Luspatercept provides meaningful and durable erythroid benefit in MDS-RS, particularly in SF3B1-mutated patients
