Background: Nirsevimab provides extended RSV prophylaxis compared to palivizumab. Methods: Prospective cohort of 840 premature infants (29-35 weeks GA) receiving nirsevimab vs. palivizumab for RSV season prophylaxis. Primary outcome: RSV hospitalization rate. Results: RSV hospitalization: nirsevimab 1.8% vs. palivizumab 4.9% (RR 0.37, p<0.001). Medically attended RSV LRTI: 3.4% vs. 8.6%. Single-dose convenience improved parental adherence (94% vs. 71%). Conclusion: Nirsevimab demonstrates superior RSV protection in premature infants with single-dose convenience, supporting its preference over palivizumab
